Candida species in community-acquired pneumonia in patients with chronic aspiration

Abstract Background When Candida species is found in a sputum culture, clinicians generally dismiss it as contaminant. We sought to identify cases of community-acquired pneumonia (CAP) which might play contributory etiologic role. Methods In convenience sample patients hospitalized for CAP, we screened “high-quality sputum” by Gram stain (> 20 WBC/epithelial cell) and performed quantitative cultures. Criteria potential role included the observation large numbers yeast forms on stain, intracellular organisms > 10 6 CFU/ml sputum. gathered clinical information that met these criteria possible infection. Results Sputum from 154 consecutive CAP had extra- with albicans , glabrata or tropicalis culture. all patients, diagnoses at admission chronic aspiration. Greater than 5 recognized bacterial pathogen ( Streptococcus pneumoniae Staphylococcus aureus Pseudomonas) other ‘normal respiratory flora’ Lactobacillus species) were present together spp. every case. Blood cultures yielded 2 cases, 1,3-beta-D glucan was 500 ng/mL 3 assayed. Since treated anti-bacterial anti-fungal drugs, no inference about etiology can be derived therapeutic response. Conclusions normal flora may contribute cause who chronically aspirate.
